The issue is intermittent but occurs more frequently if a user is adding, updating, and removing multiple service stops from a client record without saving their amendments between each change.

To avoid this issue, please ensure you save the client record between each service stop change made. For example, if a new service stop is being added, and one is being deleted, please ensure you save your changes after the new service stop is added, and save again when the old service stop is deleted.

If you have accidentally added an open-ended service stop to a client record and it needs to be deleted, ensure you first add an end date to the service stop before deleting it. Deleting open-ended service stops without ending them first has been known to cause this issue.
