These can be set up and updated on the CM portal using the client manager application. The CM Mobile application can also be used to update and amend created outcomes.
How to create outcomes, follow these simple steps:
Log in to Client Manager via Access Group UK | Login Page.
Click on the client to open their profile and click the Outcomes tab.
Click Add Outcome.
Add the title, description, and the dates required, then click Save Outcome.
The outcome can now be viewed under the outcomes tab, and here you can edit the outcome information and add an end date to close the outcome by clicking Edit Outcome. You can also add a progression note to the outcome by clicking Add Progression note.
How to add an outcome via CM Mobile:
Tap on the visit to view the visit details.
Tap the Journal tile at the top of the screen.
Tap + Add a new entry.
Tap Please pick an entry type and select Outcome progression Note.
Tap Please pick outcome to select an outcome to add a note to.
Add information to the Add note section, click Save Progression Note.
This will now be visible in the journal section of client manager and the CM Mobile application.
